The preceding deposition was taken in the matter, on the date and at the time and place set out on the title page hereof.
It was requested that the deposition be taken by the reporter and that same be reduced to typewritten form.
It was agreed by and between counsel and the parties that the deponent will read and sign the transcript of said deposition. Said jurat is to be returned, within 30 days after the transcript is made available, to the following address/e-mail:
If an errata is executed and returned to GLS within the 30 days allocated by law, the executed errata will be sent to the taking attorney for filing with the original transcript. Should an executed errata not be forwarded from GLS to the taking attorney for filing, the deposition was not reviewed and signed by the deponent within 30 days.
